Subcontractor document checklist
- W-9 or applicable country-specific tax form equivalentCurrent and completed legal tax documentation.
- Underrepresented Business Enterprise certification informationInclude applicable MBE, WBE, DBE, SBE, Section 3, or comparable certifications.
- Applicable safety rating verificationEMR, WorkSafeBC Clearance Letter, WSIB Clearance Certificate, or applicable equivalent.
- Historical safety dataProvide a three-year safety history, including relevant incident data.
- Safety programCurrent written safety policies and procedures appropriate to your trade.
- Current financial statementCPA-prepared statements are preferred when available.
- Surety informationLetter from your surety identifying single-project and aggregate bonding limits.
- Insurance informationCurrent sample certificate of insurance and additional-insured endorsement forms.
